Injectable Skin Treatments

1. Profhilo

Indications – Poor skin quality, dry skin, facial quality improvement and maintenance.

Profhilo is an innovative treatment that has become part of our routine facial maintenance. Profhilo is non-cross-linked hyaluronic acid (ie unchanged, natural) that is injected into the subdermal space, adding hydration and the all-important healing message. Hyaluronic acid is known for its anti-ageing properties such as collagen stimulation and fat pad protection. When used regularly it will maintain the hydration of the skin, the underlying quality and protect the fat pads from erosion of volume.

The healing momentum needs to be created; two treatments four weeks apart seems to be the optimum. Thereafter a treatment every three to six months is recommended.

Profhilo works well on some people but not everybody; it is not for adding volume but is a revitalising treatment. Thinner skin seems to respond better than fat, thicker skin or maybe just shows the result better. Our experience is that it works particularly well on the neck especially scraggy, crepey necks.

Profhilo is injected as approx 10 tiny injections spaced equally around the face. The neck can be included but ideally one syringe is used for either the face or the neck. Once injected it makes little bumps which can last up to two days. It does not make sense nor is easily possible to massage the bumps away; they need to go down of their own accord, which they will.

2. Juverderm Volite

Juvederm Volite can be considered as a filler for the skin; one of the amazing skinboosters together with NCTF, MHA 10 and Profhilo. These all work slightly differently and there is a crossover between results. Exactly which treatment you choose depends on your personal circumstances.

Juvederm Volite is a soft crosslinked hyaluronic acid. It is injected into the subdermis and remains for up to six months. Like all hyaluronic acids, it absorbs water and provides volume. Water encourages all natural physiological processes so by indirectly adding hydration we are encouraging naturally healthy skin and increased volume.

Juvederm Volite is injected once every six months. There can be some bruising which lasts a few days.

Juvederm volite can be painful owing to the many injections required. Numbing cream is applied for half an hour before the treatment to make the treatment tolerable.

3. Plenhyage Nucleotides/DNA

Like most treatments that we offer, Plenyhage is not new. 30 years ago, well before Botulinum toxin and fillers were available, Dr Dancey was using Salmon DNA for its rejuvenating effects on the skin. XaDeNe had a cult following and many Parisian customers would queue up for it every week. Given that there was no other safe treatment for skin quality at that time, this is no surprise.

The benefits of DNA bases as injectable products is becoming evident and we are incorporating these into our treatment regimes. DNA bases are the very structure of our DNA and contribute to the healing of tissues, organs and systems. It’s a bit like taking calcium for our bones and protein for our muscles; we need DNA bases to make up our DNA. There’s a lot about our DNA that we don’t understand but suffice to say that healing from wounds, life and damage is a DNA function. Healthy DNA allows us to repair quickly. Since ageing is a balance between damage and repair it makes sense to ensure that our healing and repair system is at its best.

We can take polynucleotides in our diet as organ meat; liver, kidneys and heart. Otherwise we can take supplements or for the best, targetted results, inject them into the very tissues that need help.

Salmon DNA is extracted from Alaskan Salmon, free from chemicals and antibiotics that our poor fish are farmed on. It is purified and available for injection as Plenhyage. It is injected into lines, crows feet, under the eyes, around the lips, into scars. Basically anywhere we need healing.

Plenhyage is not a volumising treatment but rather a healing, rejuvenating treatment similar to nutritional mesotherapy, Profhilo and PRP. It is directly comparable to PRP.

Plenhyage can sting so numbing cream is applied an hour before the treatment. It can be delivered with a needle or cannula. There may be some bruising and swelling associated with the needle.

Plenhyage is injected every three weeks for three sessions and repeated every twelve months in order to maintain the result.

4. Platelet Rich Plasma (Vampire Facial)

Platelets are involved in healing and recovery of the tissues. Platelets are the blood cells that begin the clotting process. They act together with the skin cells to create growth factors, small protein fragments that bring together all the necessary cells, nutrition, oxygen and fluid to ensure optimum healing. Because the ageing process is essentially a non-healing process, anything that improves healing will improve, reduce or reverse ageing.

Normally platelets are activated when the body is wounded, but by taking and processing blood we can activate and concentrate the platelets and their associated growth factors – hence the name Platelet Rich Plasma. This is truly natural medicine – using the body’s own response to heal lines, wrinkles, thin skin and thinning hair.

Blood is taken from the arm and is centrifuged to concentrate the platelets. The platelet rich part of the plasma is transferred into an injection syringe ready for immediate use. The PRP can be used to treat lines, wrinkles and skin quality all over the face, concentrating on the areas in need such as the eye area.

After treatment the area can swell as the many growth factors exert their effects – swelling means healing.

In our experience PRP works best for hair loss; such benefits are backed up by scientific papers and evidence.

Three treatments are recommended with four weeks in between; repeats can be every four to six months.

5. Mesotherapy

Indicated for skin ageing, anti-ageing, dry skin, fine lines, wrinkles, scars.

NCTF® 135HA is a unique, Positive Pro-ageing, polyrevitalising solution that provides the dermis with the essential ingredients it needs to thrive. It creates the optimal environment to reboost fibroblasts activity, which are responsible for the production of collagen, elastin and hyaluronic acid. NCTF® 135HA stimulates the skin without changing the facial features or expressions. Containing free HA, the New Cellular Treatment Factor (NCTF®) smoothes out fine lines, restores radiance, deeply nourishes the skin and increases density of the dermis. NCTF has been used for over 50 years as a mesotherapy solution well before the advent of fillers and botulinum toxin. NCTF is a scientifically formulated solution that maximises nutrition reaching the skin cells and is a spin-off from skin cell culture research.

The recent advance of the needles has allowed us to perform these treatments without any risk of bruising or marks. The patented nanosoft needles are made from silicon (not even metal) and penetrate the skin just 0,6mm, avoiding any risk of bruising. This means that we can perform the treatment for you at any time irrespective of your engagements.

Tiny little blobs of the highly nourishing solution are injected under the skin all over the face but concentrating on the key areas; around the eyes and mouth. The treatment is commercialised under the name “Beautifeyes”. After about three hours and certainly before the next morning the little blobs have gone as the skin imbibes its perfect nutrition.

MHA 10

This is the sister product to NCTF; 10% pure non-crosslinked hyaluronic acid. In the same way that we can inject NCTF into the skin of the face we can inject MHA 10 into the skin of the neck to concentrate on reducing neck lines and improving crepey skin. MHA 10 can also be used for cheek lines in combination with NCTF.

You will find a lot of press and commercial information about MHA 10 under the trade name Nutri Neck.

Nutritional mesotherapy combinations

We know that combination therapy works far better than monotherapy alone so have packaged mesotherapy with peeling/exfoliation and the Dermalux Tri Wave for excellent results and rapid recovery.

Device Based Skin Treatments

This is our new device for skin tightening and collagen stimulation. This RF device offers a unique possibility of non-invasive non-needle anti-ageing treatment that will give a natural longlasting result. What’s more is that the treatment can be doubled up so that two back to back treatments in two weekly sessions is all that is required.

Results always take three months to show, although changes should appear soon after the second treatment.

For those not in a hurry four treatments can be performed; one per week.

The RF energy is delivered to the tissues of the face, the neck, the body; in fact any location that requires tightening and new collagen synthesis. The energy is applied via a treatment head soaked in gel that smoothly glides over the skin. The treatment feels warm just like a hot stone massage. It’s really not unpleasant. We can treat just about any area of the face, neck or body.

The same treatment can be used to reduce fatty deposits. By simply dialling up the temperature to hot (but again bearable), the fat cells can be targetted and fat reduction achieved.

Most people can receive the Exilis device; however the treatment is not recommended for those with an implant such.

2. Microneedling

Microneedling swamped the Cosmetic field in the early 2000s; it was accepted as a medical technique rather than cosmetic. Microneedling releases huge amounts of growth factors which heal the skin, tighten the skin, reduce scars and pigmentation. It is a simple but effective anti-ageing technique.

There are different levels of microneedling. Medical microneedling uses a single-use sterile roller or pen and the needles penetrate deep into the papillary dermis. The treated skin is red for up to 12 hours but then begins to show the benefits of the treatment; increased glow, better quality, reduced scars and pigmentation. Numbing cream is applied in order to make the treatment pain free.

Home use microneedling or beauty microneedling does not go as deep and is used as a method of increasing penetration of various skin agents such as hyaluronic acid, vitamin C and growth factors. No numbing cream is used and the needles just break the skin sufficient to stimulate new growth and allow the nutrients to penetrate.

3. Endymed: Radiofrequency Microneedling

This is a huge step up from microneedling in that the needles are combined with a radiofrequency (RF) current. The current passes down the needle to reach the area beneath the skin; the papillary dermis. RF energy tightens the skin, releases growth factors and heat shock proteins all of which combine to create new collagen and elastic structures and increase to tone and quality of existing skin support fibres.

Numbing cream is applied for an hour to make the procedure tolerable and allow us to use plenty of RF energy. The face is quite red afterwards and there may be signs of needle holes where the treatment tip has been applied. This heals within 24 hours. As for all skin quality treatments, three treatments are required to achieve results. The results are visible after two sessions. Follow-up treatments are recommended every year, but can be performed every three months if necessary. We like to combine Endymed Intensif with NCTF to obtain the best results from a perfectly nourished skin.

The device that we use is the Endymed device; the specific treatment is called Endymed Intensif.

 

4. Fractional Skin Resurfacing (FSR)

This is another feature of the Endymed device; fractional skin resurfacing using RF energy. RF is a very safe energy and can be used like laser to resurface the skin but without the risk or the donwtime. Taking away small fractions of the skin allows the skin to heal quickly without complications. Three treatments are required to ensure overlap between treated and non-treated areas so that the whole surface of the skin is treated.

There is much overlap between the indications and results of the Endymed Intensif and Endymed FSR. Since they are both treating the skin and using RF energy they will both improve skin quality. However the results for the FSR are concentrated mostly on the skin surface, so skin surface issues such as poor complexion, scars, pigmentation and sundamage will respond to FSR. Skin quality and laxity can be improved more with the Endymed Intensif.

The area around the eyes is a good indication for the FSR. With specially chosen power settings the eye area can be targetted in a treatment known as Endybleph – a non-surgical eye skin improvement procedure. Numbing cream can be used for these treatments.

 

Skin Treatments

 

1. Gentle Peeling

Indications

Acne, sun damage, pigmentation, fine lines, poor complexion, dry skin. Antiageing.

Gentle peeling uses fruit-based and other natural acids such as glycolic, mandelic, lactic and azelaic to achieve a deep exfoliation, restore skin pH, hydration and sebum levels. There is no appreciable peeling, sensation or downtime with gentle peeling. We use it in combination with other gentle techniques such as NCTF or MHA 10 to enhance the results. Combining techniques achieves better and more comprehensive results. Regular gentle peeling will improve the quality and tone of the skin, smooth fines lines and irregularities, improve hydration by improving the underlying blood supply and slow down the process of ageing. Pigmentation also responds well to gentle peeling.

Salicylic acid is a gentle peel that acts specifically on the keratin component. This is good for acne, oily skin with spots and blackheads and thickened sun damaged skin.

We will select the most appropriate peel for your skin and incorporate the gentle peel into our peel-nourish-LED combination treatment.

 

2. Medium-depth Peeling

Lax skin

Medium depth peeling reaches deeper layers in the skin and will result in peeling. Because the deeper layers are touched the results will be stronger than with a gentle peel. A chemical solution is used to exfoliate the skin. The sensation is a bit stronger – say 7/10 on the heat scale – but this quickly subsides. Afterwards a healing soothing cream is applied which settles the skin and restores the skin barrier. The depth of the peel is revealed by the colour change of the skin from a pink-white frost like a dusting of icing sugar to a deeper opaque white frost. The depth is controlled by the doctor and depends on the degree of damage and the level of results desired.

Healing soothing cream is applied which allows the skin to settle immediately. On leaving the clinic your skin will have returned to a light pink colour. This is followed by a sensation of tightness and dryness as the top layers begin to separate from the lower levels. Peeling normally occurs on day 3 – 4. New skin is visible from 2 weeks. The best results are seen at three months.

As for all treatments that address skin quality, the results are long-lasting and enhance by the use of sun protection, nutritional mesotherapy and a healthy lifestyle.

3. Dermalux TriWave

This is a fantastic piece of equipment that literally re-energises the skin of the face. We can use it on the body to relieve muscle pain, after a massage and for healing. The Tri wave is as it says – a treatment option of three wave lengths to deal with just about every issue that the skin face and body will have to deal with.

Visible red light at 633nm, the optimal wavelength, stimulates the immune cells to create more skin tissue, to heal damage and to remove infective baddies.

Near infra-red light is profoundly anti-inflammatory and can combat the effects of ageing, pollution, skin disease and interventions. It soothes, cools and calms the skin.

Blue light kills the acne-producing bacteria Cutibacterium acnes. Blue light also wakes us up and stimulates our diurnal rhythm providing a much-needed counter to the winter blues.

We can select any combination of these three light sources to suit your needs and apply them in a relaxing, warming treatment that needs you only to lie down and relax. After the treatment your skin will be glowing; a typical light-induced healthy glow.

Ideally two or three treatments per week are recommended for a total of 6 – 8. We always offer this treatment after a Lanluma, combination NCTF or massage. It also features in the Libi facial. Some people just have the Dermalux as a treatment in its own right, especially those with sensitive skin or who just want that delightful glow.

4. A Word About…

Why have we not mentioned threads? Since the development of so many powerful techniques to lift, fill, stimulate and repair we have decided not to offer threads. We can achieve the same results safely, without pain or risk by using a combination of non-invasive treatments. The most effective is the Exilis RF skin tightening device, combined with some gentle volumising fillers such as the RRS Airbrush technique.
